How to Join the Carillon Gifts Program

1. Print and fill out this form to indicate the amount you want to contribute each month from your checking account to the Oglethorpe University Carillon Gifts Program.
2. Sign your name and indicate the date.
3. Return your completed enrollment form with your check for this month's gift. Your first gift will be transferred in 4-6 weeks.
4. You may increase, decrease or suspend your giving at any time by faxing us at 404.364.8500 or writing to us at:
