Information regarding the Immunization/Health
Form
The Immunization/Health Form is due
3 weeks prior to beginning of
classes. Please read and follow the directions on the immunization
form and submit it as soon as possible.
Hepatitis B vaccinations series are now required
for pre-med students and all students who are 18 years of age or
younger. Student Health Services can complete this series at school
for a small fee.
It is very important to have the month and year of
chicken pox disease or vaccination documentation on your health
record. NOTE: Don't wait too long to get both chicken pox
vaccinations. Once you turn 20 years old the fee is $74.00 each
shot! If you are 20 years of age or older, you can request a chicken
pox blood titer drawn at your physician’s office to check for
immunity.
You must read, sign and date the Meningococcal
Disease Information Acknowledgement.
Meningitis vaccinations (Menactra) vaccinations are
available at the county health department. Currently (subject
to change), for students 18 years and younger, these vaccinations
are $18.00. For anyone over the age of 18, they are $96.00.
Failure to
complete the immunization requirements will delay your class
registration and/or attendance.
