All transcript requests require a completed
Transcript Request Form.
Please note that transcript requests cannot be made via email
or phone.
Official Transcripts bearing the signature of the Registrar and the
seal of the University can be ordered as follows:
- By mailing the transcript request form to:

Request forms are available in the Office of the Registrar, Lupton
Hall
Hours of Operation: Monday, Tuesday, Friday - 9:00a to 5:00p Wednesday, Thursday - 9:00a to 6:00 pm
A $3.00 processing fee is charged for each transcript processed;
please enclose a check or money order with your request. If you
fax your request, please call us after you have sent the fax and
we can take credit card information over the phone.
The following information is also needed:
- The name under which the student attended
- Social Security Number
- Date of Birth
- The last term of attendance
- The address to which the transcript is to be mailed
- The student’s signature (Required)
